  • Visitors making inquiries:
    Visitors who use any of the forms on our website provide OB-GYN Associates with information required by us to act upon your inquiry. These include our "contact us" form and our "subscribe" form - used for sending brief messages to us subscribing to our newsletter. This information is stored internally at OB-GYN Associates and will never be shared outside of our organization.

  • Visitors referring friends:
    Visitors who use the "Forward to a Friend" form on our website provide OB-GYN Associates with their name and email address as well as their friend's name and email address. This information is used to send your friend an email invitation to view the recommended page on our website. This information is stored internally at OB-GYN Associates and will never be shared outside of our organization.

    Visitors who use any of the forms on our website provide OB-GYN Associates with information required by us to act upon your inquiry and do so willfully and with the understanding that the forms are not encrypted or HIPAA compliant.
